Today, with modern EHR systems, clinicians may have an easier time getting clinical data — but not all of it, which is a problem for providers pursuing population health goals.

The problem in particular may be the fact that EHRs have been adopted with data silos intact, from payers, pharmacies and other providers in a patients’ sphere.
